Botox or fillers
what is the difference?

← Back to the blog

The two products solve different problems. Where each one is placed, how long it lasts, and why an unnatural look does not come from the product.

This article is for information only and does not replace a medical consultation. Recommendations for your individual case are made following an in-person examination.

The two most popular injectable treatments are constantly confused, even by people who have already had them. Yet the difference is fundamental, and understanding it saves a good deal of disappointment.

Botulinum toxin targets the muscle. Filler improves volume and structure. Different purposes, different areas, different duration.

Botulinum toxin

It temporarily reduces the contraction of a specific muscle of facial expression. A line that forms with movement is smoothed out because the muscle beneath it stops creasing the skin.

It works best in the upper third of the face — the forehead, the vertical lines between the eyebrows and the outer corners of the eyes.

  • Effect after 3–5 days, full result at day 14
  • Usually lasts 3–5 months
  • No recovery period
  • Temporary but not reversible — nothing can switch the effect off immediately. If the treatment is not repeated, the muscle returns to its previous activity

Hyaluronic acid filler

It replaces lost volume and shapes contours. With age, the face does not simply sag — it loses volume and structure, because the fat compartments shrink and the bone is resorbed.

It is used in the cheeks, temples, tear trough, lips, chin and jawline.

  • Immediate result, final after about two weeks
  • Lasts between 6 and 18 months, depending on the area
  • Swelling for 2–5 days, more marked in the lips
  • Often correctable — hyaluronic acid can be dissolved with hyaluronidase, although the effect is not always immediate, complete or predictable
Being able to dissolve the product is a major advantage if the result is unsatisfactory or a complication occurs — even when the correction is not complete. This is also why permanent fillers are not recommended: with them, that option does not exist.

A simple way to tell them apart

Look in the mirror with your face relaxed.

  • If the line is visible only when you frown or smile — botulinum toxin
  • If it is also visible when your face is completely at rest — the cause may be volume loss, skin quality or a deeply set fold. Here the solution is individual: filler, skin treatment, toxin or a combination
  • If the problem is an area that has become hollow or lost its shape — filler

Why they are combined

Because most faces have both problems. A deep line between the eyebrows is caused by the muscle, whereas the nasolabial fold is caused by volume loss in the cheek.

Combining the two usually gives a more natural result than using just one of them aggressively.

The unnatural look

A frozen face and overfilled lips are not a property of the products. They are the result of an amount that does not suit the anatomy.

At the right dose, botulinum toxin softens rather than paralyses. In the right amount, filler restores volume rather than creating new volume.

That is why the choice of injector matters more than the choice of brand. The most serious complication of fillers is vascular, and it is prevented by knowledge of anatomy, not by a more expensive product.

When to seek help immediately

A vascular complication after filler is rare, but it is an emergency — the earlier it is recognised, the better the outcome. Get in touch immediately if you notice:

  • Pain that is getting worse and is out of proportion to the procedure
  • Blanching (whitening) of the skin, or mottled, net-like (livedo) discolouration in the treated area
  • Skin that feels cold to the touch
  • Any problem with your vision — blurred vision, eye pain, loss of vision
Any problem with your vision after a facial injection needs medical attention immediately — do not wait until the morning.

Practical points

  • Plan treatment at least two weeks before an important event
  • For the first 24 hours, avoid exercise, saunas, sunbeds and massaging the treated area
  • Stop blood-thinning medication a few days beforehand — only after agreeing this with the doctor who prescribed it
  • If you get cold sores often, mention it in advance when your lips are to be treated
