Why age is not the deciding factor, which three signs are, and why having surgery early does not delay ageing.
The question comes in two opposite forms. Younger people ask whether it is too early; older people ask whether they have left it too late. In both cases the answer depends on the tissues, not on your date of birth.
The three signs
The jawline. If the contour between the chin and the ear is no longer well defined and tissue is sagging over the jawline, this is the main indication.
The neck. Prominent platysmal bands, a blurred angle between the chin and the neck, and loose skin. The neck often gives away age before the face does.
The midface. Descended tissues and deepened nasolabial folds that no longer respond to filler.
If all three are present, age does not matter. If none of them is present, the operation has nothing to correct.
Why early surgery does not delay ageing
Many people believe that having a facelift earlier means ageing more slowly. That is not the case: a facelift puts the tissues back where they were at a given point in time, but it does not stop the process.
A mini-lift in someone with minimal changes gives a minimal result, which wears off within a few years. For such a patient, non-surgical treatments are often the more sensible first step — but they are not a substitute for a lift and do not do the same thing. Filler restores volume, a biostimulator works on skin quality, and a lift repositions tissue. Three different things.
Age ranges in practice
- Under 40. Surgery is rarely indicated. The problem is usually volume and skin quality, not laxity
- 40–50. Some patients already have an indication, most often in the neck. A more limited operation may be possible
- 50–65. The classic age range — the tissues are lax but still of good quality, and recovery is quick
- Over 65. There is no upper age limit. Your general health matters more than your age
What a facelift does not address
- Skin quality and texture
- Pigmentation and broken capillaries
- Fine lines around the mouth
- Lost volume in the cheeks and temples
The last point matters. A lift moves tissues back up, but it cannot bring back what has been lost. Combining it with fat grafting (lipofilling) is most often what makes the difference between a good result and a very good one.
What matters more than age
- Smoking — one of the most serious risk factors for this operation
- Blood pressure control
- A stable weight
- Realistic expectations
The aim is to look like yourself as you were years ago, not like someone else. If the result changes who you look like, the operation was too aggressive.
